About Us

Mobile Valley Physicians (MVP) is a mobile Primary Care practice where there is no office setting. The qualified Primary Care providers travel to the patient’s home or residence to see them in the comfort of their own Arizona home.
Our in-home health care providers can see patients in their private residence, Assisted Living Community, Memory Care Community, Group Home or Assisted Living Home. This makes the primary care visit comfortable for the patient and allows for a more personalized visit. There is no need to hassle with getting to an office, finding transportation or figuring out how to make it work.
In-Home Doctors and Practitioners
Our Primary Care Doctors and Nurse Practitioners are qualified and genuinely care about the patient, their needs and their health. The Providers work together with our staff, the patient, MPOA, communities, homes and outside sources (i.e.: Home Health, Hospice, Elderly Home Care and DME companies in Arizona) to ensure that the patient receives quality home care.
Home Health, Hospice, and DME:
MVP works closely with Home Health, Hospice and durable medical equipment (DME) companies to ensure continuity of care and the patients’ needs are met.

Our Staff
Our staff are experienced and dedicated to the care of our patients. The staff work with the Providers to ensure that the patients’ health care needs are met per the Provider’s plan of care. The staff will communicate with the necessary parties to ensure continuity of care is completed smoothly.
Dr. Richard Moe, MD
From Iowa originally, Dr Richard Moe attended undergraduate and graduate school at the University of Missouri-Columbia then received also a PhD in Exercise Physiology. His residency training was at the University of Iowa program in Des Moines. Following residency, Dr. Moe worked in general internal medicine and a sports medicine clinic. He later worked at Mayo Clinic in Scottsdale in general preventive medicine. Subsequently, he moved to Kansas City to work in a general medicine clinic for two years before spending the next sixteen years in Preventive Cardiology where he was residency trained. Since 2014, Dr Moe has been involved in mobile practices in assisting those who struggle to leave their homes to obtain medical care.
Dr Moe is on the following boards in: General Internal Medicine and Clinical Lipidology. Besides practicing medicine, he’s a family man, been married forty-eight year, with three married daughters and nine grandchildren.
Deborah Koval MSN, FNP
Deborah started in the medical field in 2006 completing all stages of a nursing practice while gaining professional experience in each board-certified areas such as a CNA, LPN, RN, BSN to current MSN as a Family Nurse Practitioner. These various roles within the medical field exposed her to a variety of healthcare settings including acute rehab, long-term care, psychiatric, endoscopy, telemetry, medical surgical, emergency room, trauma and intensive care unit making her a well-rounded practitioner. She joined the Mobile Valley Physicians practice in December 2019 where she continues to gain clinical experience.
Deborah is passionate about patient center care knowing that no one person is like any other. She continuously strives to focus on the individual to meet their personal needs and individualized choice on quality of life.
Nicol Montez DNP, FNP-C
Born in Okinawa, Japan, Nicol was raised an Army “brat”, finally settling in the Phoenix metroplex in 1993. She completed her Bachelor’s of Nursing degree at Grand Canyon University and worked as a Registered Nurse for 13 years, specializing in cardiology. With a love of learning and a desire to provide more for those in her care, she continued her education and graduated Summa Cum Laude from Arizona State University with her Doctor of Nursing Practice degree in family medicine. She has found her niche in providing mobile primary care. Having the opportunity to assess patients in their home, allows for a more thorough assessment of a patient’s needs and challenges. Nicol loves spending time with her husband and 4 children and resides in the east valley. She enjoys hiking, traveling, and crafts.
Dr. Mark Rotenstein
Dr. Rotenstein received his doctorate degree from The New York College of Podiatric Medicine in New York City, NY. He then completed a three-year residency at Morristown Medical Center in Morristown, NJ. After practicing in California, Dr. Rotenstein and his wife moved to Phoenix, AZ in 2021. He strives to provide quality and individualized care to his patients, and believes that education and preventative care are paramount for happy and healthy feet.
In his free time, he loves spending time with friends and family, hiking and kayaking outdoors, and exploring all the food that Arizona has to offer.
Kirsten Rivas-Andrews FNP-C
Since she was a child, Kirsten has known that she wanted to work in an area that directly touched people’s lives. From volunteering with the older generation since age 5 calling BINGO, to serving tea and cookies on weekends during college, Kirsten has always been giving back to the community and sharing her spark! After graduating with her Bachelor of Science in Nursing from the University of Arizona she deliberately worked in many different specialties such as neurology, trauma, cardiology, bone marrow transplant, and oncology, to enhance her future goal of becoming a Family Nurse Practitioner. After graduating With Distinction with her Master of Science in Nurse Practitioner from Northern Arizona University, she now provides primary care services to those in need of wellness with a smile.
Jeanne Carver MSN, FNP-C
Certified as a Family Nurse Practitioner, Jeanne’s extensive training incorporated her knowledge and experience encompassing a background of over 40 years in nursing and leadership positions, including hospital- based settings and community care. She has served in leadership opportunities through Honor Health Home Health formerly known as Scottsdale Healthcare Home Health. She currently functions as a mobile primary provider. Through all of her experiences, she’s been able to utilize her abilities and commitment to academic excellence, to positively impact patients during both an acute phases of illness, maintenance of chronic conditions, and educating them towards wellness.
Kelly Gray MSN, FNP
Kelly is a Family Nurse Practitioner with over 14 years of experience in the healthcare field. Kelly received her Bachelor of Science in Nursing from Grand Canyon University and went on to receive a Master’s of Science in Nursing and graduated Summa Cum Laude from the University of Phoenix.
Kelly’s nursing background as a Registered Nurse began in her home town at the Cleveland Clinic in Ohio, working in areas of many different specialties with a focus primarily on acute cardiac patients. She decided to move to Phoenix to open up more opportunities and began working as a Registered Nurse at Honor Health. She cared for patients on a telemetry unit with various medical conditions and also in an outpatient surgical unit. Prior to joining Mobile Valley Physicians she began her career as a Family Nurse Practitioner with Tri-City Cardiology, caring for patients with cardiovascular conditions ranging from acute and chronic coronary disease to advanced heart failure. Kelly’s philosophy of care includes being an advocate for her patients, listening to their concerns, providing education to patients and their families on health care needs, and working with them to improve their overall health and wellness.
Kelly and her husband, along with their 2 Rhodesian Ridgebacks reside in the east valley and enjoy spending time with their family and friends and being outdoors.
Amanda Robinson MSN, FNP
As an Arizona native, Amanda has always had a passion for local communities. Driven by a desire to help others, she committed her life to one of service and support within Home Health. With a focus on personalization, Amanda strives to create a trusting and comfortable environment for holistic care.
Amanda began her journey as a private in-home caregiver. During this time she enjoyed the connections with her patients and their families. Motivated to expand her impact, she went on to become a home health nurse in 2011. This new opportunity broadened her experiences to include managing the care of chronically and acutely ill patients. After home health nursing, Amanda went on to become a Certified Family Nurse Practitioner. She graduated with a Masters in Science and Nursing from Grand Canyon University in 2018. Throughout her education and tenure, Amanda partnered with a vast number of providers within the state. This familiarity with local providers, along with her professional network, ensures all patients receive the best care possible.
Today, as a mobile Family Nurse Practitioner, Amanda continues to enjoy bringing her experiences to help patients and their families get quality primary care in a way that is most convenient for them.
Dr. Deo Rampertab
Dr. Rampertab has lived in the Valley for 30 years and has enjoyed most of the recreational activities the Southwest has to offer. He is an avid hiker and mountain climber having climbed several 14,000 foot mountains and several high points in the western states. He is also an avid sailor and has recently completed the rigorous United States Coast Guard requirements for a certified Licensed Captain.
Dr, Rampertab joined Mobile Valley Physicians (MVP) in 2021 when COVID-19 made it clear that at-home care for certain high risk patients was a necessity. MVP services are also available for patients who are homebound, can not leave home unattended, do not drive, or in cases where it would take a considerable and taxing effort for the patient to leave home. MVP continues to grow and add other services and specialties available to our patients.
