Mobile Valley Physicians (MVP) is a mobile Primary Care practice where there is no office setting. The qualified Primary Care providers travel to the patient’s home or residence to see them in the comfort of their own Arizona home.
Our in-home health care providers can see patients in their private residence, Assisted Living Community, Memory Care Community, Group Home or Assisted Living Home. This makes the primary care visit comfortable for the patient and allows for a more personalized visit. There is no need to hassle with getting to an office, finding transportation or figuring out how to make it work.
All MVP staff pride themselves on communication, clarity, honesty, quality and great customer service. Our staff and Providers work side-by-side to make a smooth process for our patients, family members and care givers.
We work with the staff at Assisted Living, Memory Care, Group Homes, Assisted Living Homes and Senior Home Care facilities to insure clear communication and continuity of care for the patient.
MVP works closely with Home Health, Hospice and durable medical equipment (DME) companies to ensure continuity of care and the patients’ needs are met.
Our staff are experienced and dedicated to the care of our patients. The staff work with the Providers to ensure that the patients’ health care needs are met per the Provider’s plan of care. The staff will communicate with the necessary parties to ensure continuity of care is completed smoothly.

Our Primary Care Doctors and Nurse Practitioners are qualified and genuinely care about the patient, their needs and their health. The Providers work together with our staff, the patient, MPOA, communities, homes and outside sources (i.e.: Home Health, Hospice, Elderly Home Care and DME companies in Arizona) to ensure that the patient receives quality home care.

Dr. Richard Moe, MD
Originally from Iowa, Dr. Moe attended the University of Missouri-Columbia Medical school. He completed his residency in Internal Medicine at the University of Iowa-Des Moines and is board certified on the American boards of Internal Medicine and Clinical Lipidology. In addition, he obtained a PhD in Exercise Physiology from the University of Missouri-Columbia.
Dr. Moe began his career in a Sports Medicine Clinic and general medicine practice before joining the Mayo Clinic in the field of Preventive Medicine in Scottsdale Arizona. He went on to practice preventive medicine in a large cardiology group, focusing on preventive Cardiology and Executive Health. Dr. Moe is presently in his third year of practicing home visitation medicine in the Phoenix area. Providing medicine in the home setting is rewarding because many of his patients have less family support and need the attentive care that one-on-one visits at home provide. Providing medical care in patients’ place of living affords the opportunity to deliver care in a comprehensive way and affords the practitioner the opportunity to identify needs that otherwise may have gone unidentified.
Sharon Yenny, MSN, ANP
Sharon graduated from the University of Nebraska Medical Center with her Bachelor of Science in Nursing in 1985. She then advanced her degree and achieved her Masters in Science and Nursing as well as her Post Masters Adult Nurse Practitioner Certificate from Creighton University.
Sharon has proudly service in the United States Army Reserve for 28 years, serving as a Deputy Commander and retiring with the rank of Colonel, US Army Reserve. Sharon is Credentialed as an Adult Nurse Practitioner through the American Academy of Nurse Practitioners.
Sharon has provided Primary Care to patients in a number of settings including the Veteran's Administration Medical Center in Omaha, Nebraska and in a number of outpatient settings. Most recently Sharon has been providing Primary Care to homebound patients in the Phoenix area. She is proud to provide a vital service to the senior community, many of whom are challenged to get out to see a provider in a traditional office setting.
Dr. Andrew Straley, DPM
Andrew Straley earned his BS in biology at the University of Pittsburgh in 1995. He received his doctorate at Barry University in Miami Shores, FL. Following that he completed three years of podiatry residency at Cedars hospital in Miami, FL and South Shore hospital, Miami Shores. FL.
After completion of his residency, Dr. Straley worked in Newport News, VA for 7 years in private practice. During that time in VA, he was approached to begin seeing patients in the home for podiatry care, and through that experience, discovered his true love of delivering care to homebound patients. He moved to Scottsdale, AZ in 2012 to pursue a career in mobile podiatry, and now has a robust practice where he visits patients in private homes, group homes, senior living communities and nursing homes full time.
Nicol Montez DNP, FNP-C
Born in Okinawa, Japan, Nicol was raised an Army "brat", finally settling in the Phoenix metroplex in 1993. She completed her Bachelor's of Nursing degree at Grand Canyon University and worked as a Registered Nurse for 13 years, specializing in cardiology. With a love of learning and a desire to provide more for those in her care, she continued her education and graduated Summa Cum Laude from Arizona State University with her Doctor of Nursing Practice degree in family medicine. She has found her niche in providing mobile primary care. Having the opportunity to assess patients in their home, allows for a more thorough assessment of a patient's needs and challenges. Nicol loves spending time with her husband and 4 children and resides in the east valley. She enjoys hiking, traveling, and crafts.
Kirsten Rivas-Andrews FNP-C
Since she was a child, Kirsten has known that she wanted to work in an area that directly touched people’s lives. From volunteering with the older generation since age 5 calling BINGO, to serving tea and cookies on weekends during college, Kirsten has always been giving back to the community and sharing her spark! After graduating with her Bachelor of Science in Nursing from the University of Arizona she deliberately worked in many different specialties such as neurology, trauma, cardiology, bone marrow transplant, and oncology, to enhance her future goal of becoming a Family Nurse Practitioner. After graduating With Distinction with her Master of Science in Nurse Practitioner from Northern Arizona University, she now provides primary care services to those in need of wellness with a smile.
Jeanne Carver MSN, FNP-C
Certified as a Family Nurse Practitioner, my extensive training incorporated my knowledge and experience encompassing a background of over 40 years in nursing and leadership positions, including hospital based settings and community care. I have served in leadership opportunities through Honor Health Home Health formerly known as Scottsdale Healthcare Home Health. I currently function as a mobile primary provider. Through all these experiences, I’ve been able to utilize my abilities and commitment to academic excellence, to positively impact patients during both an acute phases of illness, maintenance of chronic conditions, and educating them towards wellness.
